Medication Consent Form
In order for PSED to administer ANY type of medication to your child, we are required by EEC School Age Regulations to have this form on file for your child.
Sunscreen Permission Form
PSED is required to have a copy of this form on file in order to apply sunscreen on your child.
Allergy Form
If your child has an allergy that must be treated with Medication, we are required by EEC School Age Regulations to have this form on file for your child.
Asthma Plan
If your child needs to manage their Asthma with Medication, we are required by EEC School Age Regulations to have this form on file for your child.
